AZUSA FOUNDATION

Provides Interim / Bridge Supportive Housing Services for homeless individuals with and without mental challenges and/or that have health conditions. The program consists of short-term housing in a safe, clean and stable environment, while accessing comprehensive case management, including basic medical services / health education, and other supportive services with meals, 24-hour supervision, with experienced staff to ensure “Whole Person Care”, that will result in housing placement ready individuals. ​
